Waiver

Each Member or Potential Member must fill out a waiver to participate in any Class or Private Lesson.

All visitors must check in with the front desk before entering class and present a photo ID when entering the facility. You must be 18 years or older to sign the waiver and become a member.

STOP: If you are under the age of 18 you will need to be present, along with a parent or legal guardian to sign for you. Online waivers will not be accepted for anyone under 18.

We have updated the wavier due to COVID-19 and all members and visitors will be expected to sign the release form before activating a new membership or returning to class. Release and Waiver of Liability & Indemnity Agreement Below.

*Minors must be accompanied by a Parent or Legal Guardian. Parents and/or legal guardian will be responsible for the supervision of children visiting the facility. If the child is disruptive to other students or the class, you will be asked to remove that child. Children are not permitted to play in the classroom area. Children are NOT to be left in a vehicle while you train!

If you are no longer a student and would like to become one, you will be required to fill out a new waiver.
